Hurrevac 2010

A couple questions for anyone that uses Hurrevac 2010:

First, how do you get international city names to appear. I have "City names" checked and I see names in the US, but nothing overseas. I see plenty in the shapefile, but they aren't appearing on Hurrevac.

Second, how do you customize/add cities to a specific lat/long? I'd like to add some military installations overseas, but I can't seem to get annotations or the city names files updated based on lat/long, but rather clicking on the map.
