Weather Forecasting Errors

Ever seen a weather map gone wrong, a weatherman messing and mixing his/her lines? If you've seen or been in some weather gone wrong, post here!!
Here are some of my instances:
1. One weather map from my local news station said Austin, TX was going to be 279F, and all surrounding cities had temperatures 200F+
2. National maps from the NOAA stated Tropical Storm Alex (Later hurricane) was a hurricane, meaning they ignored their own analysis on Alex and went forward and upgraded Alex on the national maps.
3. The Weather Channel's 3-day forecast bar that was supposed to be on the bottom of the screen was in the middle, effectively covering Stephanie Abrams and Mike Bettes.
