Where did this system originate? It passed ACK on the 11-13th of December, where was its demise? did this system have an eye feature?

Believe it or not, it can be traced back to Super Typhoon Gay in the Western Pacific. It was known as the "Storm That Wouldn't Die".

When Ex-Gay went through AZ we had heavy rains and strong winds from it. Gay is listed as the second strongest STY on record at 872mb. The storm originated near the dateline bombed out as it headed towards Guam but weakened drastically as it passed over Guam. It reintensified as it recurved near Okinawa (unusually far north for a late Nov/Dec storm) became ET south of Japan and hitched a ride on an El Niño inspired Pacific Jet and took off like a scalded rabbit into the US West Coast over CA. The heavy rains in December 1992 led into what was to be an extraordinarily Wet January in 1993 and major flooding in AZ.
